Here is what will be needed for the next step:

A copy of your ID (If you have not already sent this to us)

And copies of the following documents for your child if you have them:

Birth Certificate (If you have not already sent this to us)
Recent picture of your child
Immunization card
Social Security card
Medical card

You can just take a picture with your phone and text to our private line at 800-923-6784 anytime.

Your coordinator will also be asking for names of any daycare, preschool, or school your child has attended.
